School Nurse

June 2024 – Physical/Health Form Requirements

As we continue planning for the upcoming school year, I wish to share with you the Physical and Immunization Requirements for admission of students.
Physical/Health Form Requirements
Completion of the Commonwealth of Virginia School Entrance Health Form  is required for all Pre-Kindergarten, Kindergarten, and Transfer Students. The Health Form must be completed and on file in our school office prior to the start of the new school year. 7thgrade students are required to have an updated immunizations record submitted to the school clinic prior to the start of the school year.
Completion of the Confidential Health History Form is required at the beginning of each school year for returning students in Kindergarten through 8thGrade, and is completed by the parents. Please annotate any changes in your child’s health history, including updates in allergies, even if they do not have anaphylactic reactions.
A School Physical is only required for the first day of school in grades Pre-K and Kindergarten, but is requested after your other child/ren has had their annual health physical so that we can maintain up to date records for your child’s health and safety.
Immunization Requirements
Immunizations required by the Virginia Department of Health can be found on Supplemental Guidance for School Required Vaccines.

Vaccines Required Prior to 7th Grade:
  • Meningococcal Conjugate (MenACWY) Vaccine
  • Diphtheria, Tetanus, & Pertussis (Tdap)
  • Parent’s or guardian’s may elect for their child not to receive Human Papillomavirus (HPV) Vaccine. A discussion with your child’s pediatrician is suggested.
Kindergarten Vaccine Required Prior to School Entry                 
  • Hepatitis A

Please be aware that providing documentation of immunization is a necessary condition for enrollment at All Saints. Those students who have not received proper immunization will not be permitted to attend school at the start of the 2024-2025 school year.

Health forms need to be submitted to the School Nurse for all students by August 1st to provide time for the nurse to review and request any additional information. 

NOTE: After your child completes his/her Annual Physical with their physician, ALL parents are asked to submit the COMMONWEALTH OF VIRGINIA SCHOOL ENTRANCE HEALTH FORM completed by your child’s pediatric office. Please annotate ALL medical conditions on medical forms for your student’s safety. 

If your student has medications that need to be taken at school, either for a short period of time, in case of emergencies, or for daily administration, please find the appropriate forms referenced below.  

NOTE: ALL medications must be hand-delivered by an adult to the school nurse with the associated form completed and signed. Students are not allowed to self-carry medications in elementary school, with a few exceptions by permission of their physician.

Physical & Immunization Requirements

  • Completion of the Commonwealth of Virginia School Entrance Health Form (fillable pdf version) required for all Pre-Kindergarten, Kindergarten, and Transfer students. The Health Form must be completed and on file in our school office prior to the start of the new school year.
  • Virginia has passed legislation requiring additional vaccinations for school admittance effective July 1, 2021. Required Immunizations by the Virginia Department of Health can be found on Supplemental Guidance for School Required Vaccines.

    NEW Vaccines Required Prior to 7th Grade:

    • Meningococcal Conjugate (MenACWY) Vaccine
    • Human Papillomavirus (HPV) Vaccine at the parent’s or guardian’s sole discretion, may elect for the child not to receive the HPV vaccine.
    • Diphtheria, Tetanus, & Pertussis ( Tdap)
