June 2026 – Physical/Health Form Requirements

Physical/Health Form Requirements

Llenar el Commonwealth of Virginia School Entrance Health Form  is required for all Pre-Kindergarten, Kindergarten, and Transfer Students. The Health Form must be completed and on file in our school office prior to the start of the new school year. 7tograde students are required to have an updated immunizations record submitted to the school clinic prior to the start of the school year.

Completion of the Confidential Health History Form is required at the beginning of each school year for returning students in Kindergarten through 8to Grade, and is completed by the parents. Please annotate any changes in your child’s health history, including updates in allergies, even if they do not have anaphylactic reactions.

A School Physical is only required for the first day of school in grades Pre-K and Kindergarten, but is requested after your other child/ren has had their annual health physical so that we can maintain up to date records for your child’s health and safety.

Immunization Requirements

Immunizations required by the Virginia Department of Health can be found on Orientación suplementaria para las vacunas requeridas por la escuela.


Vaccines Required Prior to 7th Grade:

  • Vacuna conjugada meningocócica (MenACWY)
  • Tetanus, Diphtheria, & Pertussis (TDaP)
  • Human Papillomavirus (HPV) Vaccine – Parents or guardians may elect for their child not to receive the HPV vaccine at this time.  A discussion with your child’s pediatrician is suggested.
Kindergarten Vaccine Required Prior to School Entry                 
  • 4 Doses of Diphtheria, Tetanus, & Pertussis (DTaP)
  • 2 Doses of Polio (IPV)
  • 2 Doses of Varicella (Chicken Pox)
  • 2 Doses of Measles & Mumps
  • 1 Dose of Rubella 
  • 3 Doses of Hepatitis B
  • 2 Doses of Hepatitis A

Please be aware that providing documentation of immunization is a necessary condition for enrollment at All Saints. Those students who have not received proper immunization will not be permitted to attend school at the start of the 2026-2027 school year.

 

Los formularios de salud deben enviarse a la enfermera escolar para todos los estudiantes antes de Agosto 1ro para dar tiempo a la enfermera para revisar y solicitar cualquier información adicional. 

IMPORTANTE: Después de que su hijo complete su examen físico anual con su médico, se les pide a TODOS los padres que envíen el FORMULARIO DE SALUD PARA LA ENTRADA A LA ESCUELA DE VIRGINIA completado por la oficina de pediatría de su hijo. Por favor anotar TODAS las condiciones médicas en formularios médicos para la seguridad de su estudiante. 

If your student has medications that need to be taken at school, either for a short period of time, in case of emergencies, or for daily administration, please find the appropriate forms here.  

IMPORTANTE: TODAS las medicinas deben ser entregados en mano por un adulto o la enfermera de la escuela con el formulario asociado completado y firmado. Los estudiantes no pueden llevar consigo medicamentos en la escuela primaria, con algunas excepciones con el permiso de su médico.
